Rachelle Desbiens Gray's Role

  My services have been retained by your Insurance Company to offer you assistance in a rehab program. This assistance varies from person to person depending on the situation, but can include any of the following:

Medical management, i.e.., meeting with your Physicians, coordinating treatment such as Physiotherapy, etc.;
Work with you to set realistic Employment goals
Communicate with your Employer
Look at your Transferable Skills if you need to change your  occupation for medical reasons;
Coordinate a placement if required in order to obtain on the job training;
Provide you with Job Search Strategies, Resume writing, Community Resources, etc.

Your Responsibilities While Involved In Rehabilitation

  Your involvement and commitment in the rehab program is essential in order to obtain the best results. Rehab services are offered to you for a specific time frame.
I will work closely with you, and your participation is "key" in formulating the best rehab program for you.

  While involved in your rehabilitation program the following are your responsibilities:

Notify my office of any time you will be away during your rehab program;
Be available to meet with me at your home if possible as required;
Attend scheduled appointments and notify my office of any cancellations;
Follow through on items discussed, i.e., checking our resources, making phone calls, etc.;
Call into the office on a weekly basis or any time there is new information that would affect your rehabilitation program;
Participate fully and be open and honest in your communication with me while discussing your employment options and plans;
Call the office if you have any questions, suggestions, ideas that may   impact on your rehabilitation program;
Please call your Claims Adjudicator for any questions regarding your Long Term Disability Payments;

  Once the claimant has been medically released to return to work and the type of employment and salary expectation are determined, the consultant and the client will begin job search. The worker will assist the client in the preparation of realistic job search techniques including resume preparation and/or refer the claimant to a job finders club.
  Rachelle will approach and discuss employment opportunities for the client with prospective employers by means of telephone calls and meetings. Once employment is secured for the claimant, support by means of counseling and monitoring for a pre-determined period of time can be negotiated between the client, employer and LTD carrier.
  This continuity is often essential to ensure a successful job placement.
